Crossroads Family Medicine is pleased to offer to all its uninsured patients the opportunity to become flat-rate patients. This payment method is unique and may enable patients to contain many of the costs associated with their primary care.
The contract between Crossroads Family Medicine and its flat-rate patients fully describes services that are covered by the this fee. Below is a brief outline of this coverage followed by a summary of the flat-rate pricing schedule:
Crossroads Family Medicine services covered by the annual fee: · Annual complete physical exam · Sick visits · Screening tests offered in the clinic (i.e. Strep, Influenza, Pregnancy) · Lab tests offered in the clinic (i.e. urinalysis, glucose, lipids, AST/ALT, A1c, urine microalbumin) · Blood draw and specimen collection in clinic · Administration of all immunizations and injections · Procedures offered in the clinic (i.e. pulse oxymetry, ECG, spirometry, nebulizer treatments, earwax removal) · Surgeries offered in the clinic (i.e. treatment and excision of skin lesions, laceration repairs) · Phone access to physician 24/7 · Email access to physician
Services NOT covered by the annual fee: · Certain clinic supplies (i.e. braces, slings, casting) · Immunizations and injections · Pathology (i.e. reading of PAP, tissue examination) · Hospital (i.e. inpatient, Emergency Department, surgery) · All specialist services with or without referral · Radiology (films and interpretations) · Labs tests and screening tests not processed in the clinic (send-outs) · Medications and pharmacy supplies
In summary, the flat-rate fee covers virtually all services performed at Crossroads Family Medicine for which we set prices and collect fees. The flat-rate fee does NOT cover services performed by hospitals, clinics, pharmacies, or other centers for which Crossroads Family Medicine has no control over pricing or delivery of services, whether or not Crossroads Family Medicine indirectly charges for these services (such as blood work sent out for testing, but charged through the clinic for patient convenience).
