Crossroads Family Medicine is a privately owned primary care clinic in Concord, New Hampshire offering a full scope of primary care medical services to both insured and uninsured patients.
Crossroads Family Medicine accepts most private insurances and is in-network for the following health plans: ANTHEM, CIGNA, HARVARD PILGRIM, AETNA, UNITED HEALTH, and others. Crossroads Family Medicine also accepts MEDICARE, but is presently closed to new Medicaid patients. The clinic is able to offer patients not covered by insurance either a low monthly flat-rate fee that covers nearly all charges incurred within the clinic or a income based sliding-scale fee (paid at the time of each office visit).
It is important to note that regardless of the method of payment there is no difference in the quality of patient care at Crossroads Family Medicine. Each patient regardless of how they pay for their services will be offered the same level of care according to accepted medical “standards” at any given time.
What is different at Crossroads Family Medicine is the level of access to care. As a patient of Crossroads, we will do our best to give you 24-7 access to your primary care provider. When able, the clinic will be opened off-hours to care for your special needs. Your provider, in reality, works for you!
To make Crossroads Family Medicine your medical home, please call the clinic to pre-register and setup an appointment to visit with your new primary care provider.
